Posters in this thread have referenced two different aspects of the rules. The basic overthrow rule for a ball that goes out of play is on page 49 of the rule book under 8.4(10) D. It is the one B.J. quoted:
“When the ball is in play and is overthrown (beyond the boundary lines) or is blocked: EFFECT: All runners will be awarded two bases, and the award will be governed by the positions of the runners when the ball left the fielder’s hands. If two runners are between the same bases, the award is based on the lead runner.”
Note that this rule does not mention whether an outfielder or infielder made the throw.
Stick8, you quoted a rule a little further down that deals with a thrown ball that hits a runner or umpire and then goes out of play. That is in section E. The first part of that rule notes that the “ball is dead, and all runners are awarded two bases.” Subsection 1 adds details about whether the throw came from an infielder or outfielder. For an infielder, the award of bases is “governed by the position of each runner at the time the pitch was made; however, if all runners, including the batter-runner, have advanced at least one base when the infielder makes the wild throw, on the first play after a pitch, the award shall be governed by the position of the runners when the wild throw was made.”
Umpires should glance at all runners as a throw is being made so that the proper rule can be enforced if the thrown ball goes out of play.
In the case cited at the start of this thread, the runner should be awarded third base.

It doesn’t matter what direction the runner is going. Even though he needs to tag first base (or be called out on appeal), he is awarded two bases from the time the ball left the fielder’s hand. In this case, he is awarded third base because he already is past first base. (Yes, this is the USA rule, too.) If he fails to retouch first base, the defense may appeal him for leaving the base too soon.

Let’s see if I can answer the key questions. 1. When do you call the out on the runner from 2B who did not tag? If he had recrossed the commitment line, you would call the out as soon as that happens. In this case, he stood near the scoring line, so the out is not called until the defense properly appeals. 2. When was 2B available for the runner from 1B? As soon as it was clear that the runner from 2B had “given up” that base, which happened when he touched 3B. Runner from 1B could have stepped on 2B and been safe even during the live ball appeal. 3. Does the defense have to make a verbal appeal when throwing back to 2B on this live ball appeal? OK, there’s a lot of discussion on this. Bottom line, it depends on what the defense says and does. For example, if someone yells, “He didn’t tag at second,” and then the defense throws to 2B, I have an out when the defense touches the base. But if no one says anything, as an umpire I hesitate to see what play they defense is making. Usually, someone will say they are appealing. Perhaps this live ball appeal language from umpirebible.com explains it best:: “The appeal must be explicit. A defensive player with the ball must then either tag the player whose action is being appealed, or touch the base at which the appealable action took place, and then must indicate to the umpire either verbally or with an unmistakable action the intention to appeal. An appeal should be clearly intended as an appeal, either by a verbal request by the player or an act that unmistakably indicates an appeal to the umpire."

Great explanation from fresnomdn. There is no special senior rule for this play. The runner gets 3B. (Does not mater if he had tagged or not; he still gets 3B. In the case where a runner fails to tag, the defense can appeal that mistake.)

ISSA follows USA (formerly ASA) Softball rules for most of its rules, including the situation MurrayW describes here. SSUSA and USA rules are the same for this situation. Tim cited that rule in his post. So the umpire should NOT have awarded the 2B and 3B runners a base.
