Do I have to have a replacement card issued to me or is my picture ID good enough due to me already being registered and on my teams roster? Please advise. Thank you, Jim Perry |

If you want an actual new card, the order form is available here from the MENU BAR above at FORMS • Player Registration ... Check the box marked replacement, fill it in and send the replacement fee ... |
