What rule covers a batter hitting a pitch that bounces over the plate.

Rulebook §6.15 • SHORT PITCH on page 37: "A pitched ball hitting the ground in front of home plate is a dead ball. A ball shall be called on the batter."

What if the batter swings and hits the ball fair?

Nothing ... The batter can't put a "dead ball" in play ... It's an additional "ball" in the count and the at-bat resumes as if nothing had happened ... Unless that's "ball four", in which case the batter is awarded first base on the walk ...
