| Sept. 3 SSUSA Staff 3505 posts | SSUSA has added a standard Player Emergency Information Form to it's Forms page of this web site. We encourage all players to download this form, fill it out and provide it to your team's Manager. Managers are also encouraged to require this form from their players, and keep them with other essential team administration paperwork, especially when the team is practicing or competing in any event.
